Pictures of my child may be used in future VBS promotional material
I/we hereby give my permission for the individual listed above to ride the All Nation Baptist Church van, or a leader’s vehicle to attend VBS. I understand that my children will be under adult supervision. I further understand that in signing this permission slip, I release and hold harmless All Nations Baptist Church. By signing this permission slip, I release and hold harmless its trustees, officers, employees and any volunteers from any liability, past or future, fully and completely. I authorize the staff or designated medical professionals and/or volunteers to administer emergency medical assistance if I cannot be reached.
The cost for the T-Shirt and CD Combo is $15 per child (with a max of $25 per family).
This combo is not required to attend VBS. Please note that children without a registration fee will not receive the t-shirt and cd.
Payment can be made in person, mail, or electronically. (If paying by check, please make all checks out to: ANBC with VBS in the memo line).

Payment can also be done via Venmo (@IC-ANBC) with VBS and the name of the child(ren) in the "What's this for?" box.)
Please choose the T-shirt size for the child (if you are getting the T-shirt/ CD combo). If the size is not listed above, please mark "other" and let Pastor Tim know the size.
